IVUS Versus Angiography-Guided Drug-Eluting Stent Implantation: The ULTIMATE Trial

The ULTIMATE trial randomized 1,448 all-comer patients undergoing drug-eluting stent implantation to IVUS-guided versus angiography-guided PCI, measuring target vessel failure (TVF — a composite of cardiac death, target vessel MI, and ischemia-driven target vessel revascularization) at 12 months as the primary endpoint.

IVUS guidance significantly reduced TVF at 12 months: 2.9% versus 5.4% in the angiography group (HR 0.530, p=0.019). The benefit was concentrated in patients who achieved optimal IVUS criteria for stent deployment — minimum stent area ≥5.0 mm², plaque behind stent <55%, and no major edge dissection — demonstrating that the IVUS advantage depends on using the imaging information to optimize deployment, not merely recording images.

ULTIMATE is the definitive RCT establishing IVUS-guided PCI’s superiority over angiography in an all-comer patient population. The ADAPT-DES registry (Witzenbichler et al., 2013) provided observational evidence; ULTIMATE provides randomized confirmation. The magnitude of TVF reduction — approximately 50% — is clinically significant and consistent across the subgroup analyses.

The trial establishes that IVUS guidance is not merely a quality assurance tool for complex cases but provides meaningful outcome benefit when used to achieve and verify defined optimization criteria across routine DES implantation cases. European guidelines now give IVUS-guided PCI a Class I recommendation for left main and complex coronary interventions.

We rate the evidence strong. The ULTIMATE RCT in 1,448 patients establishing that IVUS-guided DES implantation reduces target vessel failure by ~50% compared with angiography guidance — definitive randomized evidence supporting IVUS guidance as the standard for drug-eluting stent implantation.
