In advanced heart failure, decisions get high-stakes and the margins thin — and this review argues that direct pressure measurements inside the heart are how you navigate them.
Right heart catheterization, the authors hold, is central to managing the sickest heart-failure patients: diagnosing the problem, stratifying risk, and above all guiding the big decisions around mechanical circulatory support and transplant evaluation, where the hemodynamic numbers are decisive in ways non-invasive estimates can’t match.
The practical takeaway is to keep the procedure in active use for complex cases rather than defaulting to non-invasive surrogates when the stakes justify direct measurement. The authors expect little resistance beyond a general preference for avoiding invasive tests where alternatives exist.
We rate the evidence low-to-moderate: a balanced 39-reference narrative review from a reputable group, but not a systematic synthesis. Its clinical significance is moderate and population-specific — invaluable for the relatively small group of advanced-heart-failure patients headed toward devices or transplant, less broadly relevant than the heart-failure label alone suggests.
